Does anyone know where the template is for "Take A Number" (table numbers) from Weddings Fall 2008. I can't find it.

I have just spent an hour looking for "Taking a Bough" template I found on the Martha Stewart Weddings - Summer 08 Magazine on page 62. And I still havent found it - does anyone know where or if I can find it???
(I would really like it for our ceremony)
